Over the years I have used versions of these forms and they have worked just fine for me so they should be fine for your needs. A small word of warning, each state has slightly different laws, due to these variations, you mght need to add a paragraph or two to the forms to make them completely compliant with your local laws. However, on the whole these forms should do the trick.
